Help finding provider

To make it easier for you to find an individual provider, clicking on the "Help finding provider..." link on the search panel will bring up the following form:

You can type part of all of a provider name/address in the form as shown above. You don't have to put something in both the name and address, but doing so can give you a powerful way to search the list of healthcare providers.

Example searches

• Type "primary care trust" in the provider name box, and "essex" in the provider address box to list all Primary Care Trusts in Essex.
   
• Type "ipswich" in the provider address box, and choose "independent" from the organisation type drop down list to show all independent healthcare providers in Ipswich.
   
• Type "dialysis" in the provider name box, and choose "independent" from the organisation type drop-down list to show independent dialysis centres (with the word dialysis in the healthcare provider name).

Selecting a provider

To select a healthcare provider, highlight one in the Results list and click on the Select button - your chosen provider will be placed in the healthcare provider drop-down list in the "Search for visits" area to the left of the screen.
